Fill in the data fields. In the field that asks for "Justification," please explain your relationship to CWID AND, if appropriate, indicate your status an an Interoperability Trial Submitter. When finished, submit the form.

The CWID-Joint Management Office will review your request for access. Upon approval, you will receive an e-mail notice containing your user id and password for the main portion of the CWID web site and a separate user id and password for the legacy portion of the web site where the Trial Survey information is maintained. Enter the CWID web site, fill in your user ID and password  and then enter the password-protected side of the web site. On the home page of the main site you will see a "Trial Surveys" tab on the top navigation bar. Click this tab and use the second user id and password we provide to proceed with registering your trial.
